Diabetes Management Program

Emory's Diabetes Management Program is comprised of two comprehensive patient-centered care program modules developed to improve clinical care and reduce acute and chronic complications in people affected by Type 2 Diabetes.

Each program is designed to provide patients with high blood sugar levels the knowledge, skill, and ability they need to better manage their diabetes. We ask patients to participate for one full year as we work together to help them reach their long-term healthy blood sugar level goals.

Diabetes Management Program

First Meeting

Office visit with a nurse practitioner, certified diabetes educator, and board-certified diabetes manager

  • We will review current medicines, what they do and why they are important and whether you need a change of medication. We will discuss any issues that make it hard for you to bring your blood sugar down and work on solutions.

Second Meeting

One-week follow-up with the diabetes educator by telephone or patient portal (communication via a secure online website).

  • You will provide one week of blood sugar numbers to review with the diabetes educator.
  • We will talk about diet education and any concerns or questions you may have from your first appointment.

Third Meeting

Diabetes Education – A one-hour, one-on-one education session with a registered dietitian/certified diabetes educator

  • Healthy Eating 
  • Problem Solving
  • Reducing Health Risks
  • Healthy Coping 
  • Taking Medication
  • Being Active
  • Monitoring Blood Sugar

Ongoing Diabetes Management and Follow-Up

We will continue telephone or portal communication with the dietitian and in-person office visits with the nurse practitioner for the following year. Active participation in this program is important for the best results in improving your blood sugar levels. Our goal is for you to live your life to its fullest and healthiest.

Graduation

One year from your first visit, if your blood sugars are in a healthy range

Diabetes Self-Management Training Program

The Self-Management Training Program is recognized by the American Diabetes Association. We are honored that you and your doctor have chosen us to help you learn more about how to successfully manage your diabetes. The American Diabetes Association has developed a three-session program that focuses on seven key areas.

Sessions 

1. “One-on-one” Evaluation – First, you will meet with a certified diabetes educator for one hour. During this meeting, we will ask you how you currently manage your Diabetes and provide you tips to help you. 

2. Group Class – The next step is to attend a group class at one of the many Emory Healthcare locations across Metro Atlanta.

The group classes include education and discussions about:

  • Healthy Eating 
  • Problem Solving
  • Reducing Health Risks
  • Healthy Coping 
  • Taking Medication
  • Being Active
  • Monitoring Blood Sugar

The group setting is a very supportive environment where lots of questions are answered and you and your classmates have a chance to participate in the discussion.

3. Follow-Up Group Class – You will be automatically scheduled for a one-hour follow-up class, which will take place two months after you finish the group class. Please bring a copy of your blood glucose logs and A1C to this class, if available.

Attending all three sessions is important for the best results in improving your blood sugar levels. Our goal is for you to live your life to its fullest and healthiest!
